PRIVATE MEMBERSHIP AGREEMENT

Major Roots Holistics Private Membership Association

Welcome to The Rooted Life, a private 12-week educational wellness program offered through Major Roots Holistics Private Membership Association.

The Rooted Life is designed for members who want a deeper level of personalized guidance, education, accountability, and support while exploring the foundations of their wellness.

By signing this Agreement, I am voluntarily requesting membership participation and access to the private educational wellness services offered through Major Roots Holistics Private Membership Association.

1. PRIVATE MEMBERSHIP

I understand that Major Roots Holistics operates as a private membership association.

By entering into this Agreement, I am voluntarily choosing to participate as a private Member and receive educational information, wellness guidance, private resources, and other Member services within the Association.

Membership is voluntary and may be offered through application, invitation, approval, or private agreement.

This Agreement creates a private contractual relationship between the Member and the Association.

Membership and program access are personal to the Member and may not be transferred to another person.

I understand that participation is voluntary and that I remain free to decide whether or not to follow any information or educational suggestions shared with me.

2. PURPOSE OF THE ROOTED LIFE

The Rooted Life is a 12-week private educational wellness program designed to provide a more involved level of guidance, education, accountability, and support.

The Association provides a private and respectful setting for Members seeking foundations-focused wellness education.

Educational discussions may include areas such as:

  • Nourishment and food choices

  • Hydration

  • Mineral patterns and mineral awareness

  • Digestion

  • Lifestyle habits

  • Stress and nervous system support

  • Sleep and daily rhythms

  • Movement

  • Herbs and traditional wellness practices

  • Nonprescription supplements

  • Pantry and food education

  • Other foundations of general well-being

The Association honors personal responsibility, freedom of choice, privacy, peaceful association, and the Member’s right to make informed decisions regarding food, lifestyle, wellness practices, outside care, and personal beliefs.

Spiritual or faith-centered encouragement may also be shared when welcomed by the Member.

3. WHAT IS INCLUDED

The $2,750 membership contribution for The Rooted Life includes participation in the complete 12-week program.

One Hair Tissue Mineral Analysis — HTMA

One HTMA test and laboratory kit are included unless otherwise agreed to in writing.

The Member will receive HTMA preparation, collection instructions, and educational support.

Weekly Private Sessions

The Rooted Life includes one private Member visit or session each week during the 12-week membership period, subject to scheduling and the terms of this Agreement.

Weekly sessions are intended to provide:

  • Education

  • Guidance

  • Accountability

  • Clarification

  • Personalized wellness support

  • Review of progress and challenges

  • Discussion of foundational wellness practices

  • Ongoing adjustment of educational suggestions as appropriate

Additional Brief Support

The Member may receive up to one additional 15–20 minute support conversation per week, if needed and available.

This additional support is intended for brief clarification or support between regular sessions.

Additional support is subject to availability, does not accumulate or roll over, and does not extend the 12-week membership period or create a refund or credit.

Private Educational Support

The program may also include:

  • Private educational materials

  • Resources

  • Member support

  • Food and nourishment education

  • Hydration and mineral education

  • Digestion education

  • Stress and sleep education

  • Movement and lifestyle education

  • Herbal education

  • Educational suggestions based upon information voluntarily shared by the Member

  • Educational review of HTMA mineral patterns when available

4. A FULL 12-WEEK GUIDED PROGRAM

I understand that The Rooted Life is designed to provide a more involved and consistent level of support than a self-directed or limited-session program.

The Member and practitioner will work together throughout the 12-week period through weekly private visits, education, communication, and accountability.

I understand that the Association provides education, guidance, accountability, and support, while I remain responsible for my personal participation, choices, and follow-through.

The program is intended to build upon information learned throughout the 12 weeks.

I understand that consistent participation may help me receive the greatest educational benefit from the program.

Participation does not require perfection.

If something feels difficult, unavailable, unaffordable, unrealistic, unclear, or inappropriate for me, I am encouraged to communicate that so the educational approach can be discussed.

5. HAIR TISSUE MINERAL ANALYSIS

I understand that Hair Tissue Mineral Analysis is used within Major Roots Holistics as an educational wellness tool.

HTMA may provide information regarding mineral levels, ratios, and patterns present in the submitted hair sample.

The HTMA may be used to provide general educational insight into mineral patterns and lifestyle-support considerations.

I understand that HTMA is not used to diagnose, treat, cure, or prevent disease.

HTMA results are reviewed for educational and wellness purposes only and are not a substitute for medical laboratory testing, diagnosis, treatment, or advice from a licensed healthcare professional.

Results may be affected by:

  • Sample collection

  • Hair treatments

  • Environmental exposure

  • Laboratory methods

  • Sample quality

  • Other individual factors

I agree to follow the laboratory’s collection instructions carefully and ask questions before collecting my sample if anything is unclear.

I understand that delayed, incomplete, contaminated, or unusable samples may delay the educational review and do not automatically extend the 12-week membership period.

6. EDUCATIONAL NATURE OF SERVICES

I understand that Major Roots Holistics provides private educational wellness information and lifestyle-based Member support.

Major Roots Holistics and its practitioner do not provide:

  • Medical diagnosis

  • Medical treatment

  • Emergency medical services

  • Mental health treatment

  • Disease management

  • Prescription services

  • Medication changes

  • Medical nutrition therapy

  • Services intended to diagnose, treat, cure, or prevent disease

Major Roots Holistics does not represent itself or its practitioner as a licensed physician, dietitian, pharmacist, mental health professional, or other licensed healthcare professional unless specifically stated otherwise in writing.

Information may include discussion of food, nourishment, minerals, herbs, nonprescription supplements, hydration, movement, digestion, sleep, stress, lifestyle practices, traditional wellness practices, and other areas of general wellness.

I understand that these discussions are educational in nature.

Nothing provided through The Rooted Life is intended to replace care from a licensed or appropriately qualified healthcare professional when such care is needed.

7. MEMBER PARTICIPATION AND RESPONSIBILITY

I understand that The Rooted Life is an active educational partnership.

The Association provides education, guidance, accountability, and support. I remain responsible for participation, personal choices, and follow-through.

I agree to make a good-faith effort to:

  • Complete intake forms and questionnaires honestly, thoughtfully, and as fully as reasonably possible.

  • Attend scheduled weekly visits and arrive prepared to participate.

  • Communicate openly about questions, concerns, progress, setbacks, stressors, financial or access barriers, and steps that feel unrealistic or unclear.

  • Review educational materials.

  • Complete mutually discussed reflection, tracking, or lifestyle steps to the best of my ability.

  • Collect and return my HTMA sample in a timely manner according to laboratory instructions.

  • Ask for clarification before acting on information that is confusing or incomplete.

  • Treat the practitioner, Association, and other Members respectfully.

  • Protect private conversations, materials, and Member information.

I understand that limited participation, repeated missed visits, incomplete forms, delayed testing, or lack of communication may reduce the support and educational progress available to me during the fixed 12-week program.

I understand that I am responsible for providing accurate information to the best of my knowledge regarding medications, supplements, allergies, sensitivities, diagnosed conditions, pregnancy or breastfeeding status, prior adverse reactions, and other circumstances relevant to the educational support requested.

8. MEDICAL CARE, OUTSIDE CARE, AND EMERGENCIES

I understand that I remain responsible for my own health, wellness, and personal decisions.

I am free to accept, decline, or modify educational suggestions provided through the Association.

I agree not to begin, stop, or change a prescription medication or medical treatment based solely upon information shared by Major Roots Holistics.

Medication and medical-treatment decisions should be discussed with an appropriately licensed healthcare professional.

The Rooted Life does not provide emergency or urgent medical care.

If I experience severe, urgent, worsening, or concerning symptoms, including but not limited to:

  • Chest pain

  • Difficulty breathing

  • Fainting

  • Severe pain

  • Heavy bleeding

  • Signs of stroke

  • Thoughts of self-harm

  • Any condition I believe may constitute an emergency

I understand that I am responsible for seeking prompt and appropriate medical or emergency care.

Membership in Major Roots Holistics does not prevent or discourage me from seeking outside medical or professional care.

9. NO GUARANTEES

I understand that every individual is different and that outcomes cannot be predicted or guaranteed.

The Association makes no guarantee of specific:

  • Physical outcomes

  • Emotional outcomes

  • Financial outcomes

  • Health outcomes

  • Laboratory outcomes

  • HTMA outcomes

  • Wellness outcomes

Results and experiences may depend upon participation, consistency, personal choices, lifestyle, outside factors, HTMA sample timing and quality, stress, environment, and circumstances outside the Association’s control.

Testimonials, examples, educational stories, or descriptions of another Member’s experience are not promises or guarantees that I will experience the same result.

No specific result, improvement, or outcome is promised or guaranteed.

10. PROGRAM CONTRIBUTION

The membership contribution for The Rooted Life is $2,750.00, unless a different written contribution or payment arrangement is accepted by the Association before enrollment.

This contribution provides access to the complete 12-week Rooted Life experience and the services specifically described in this Agreement.

The contribution includes reserved practitioner time, program space, preparation, private educational resources, Member support, weekly visits, and the included HTMA.

Unless specifically stated otherwise in writing, additional testing, products, supplements, herbs, outside professional services, or other expenses are separate from the Rooted Life contribution.

11. CONTRIBUTION AND NO-REFUND POLICY

Once the contribution is submitted and enrollment is accepted, contributions are final and non-refundable to the fullest extent permitted by applicable law, except where Major Roots Holistics Private Membership Association expressly agrees otherwise in writing or where a refund is required by applicable law.

This policy reflects the reservation of practitioner time, program space, preparation, educational materials, private resources, Member support, weekly visits, and the included HTMA test.

Refunds or credits are not issued because of:

  • Missed visits

  • Missed support conversations

  • Unused support

  • Scheduling conflicts

  • Change of mind

  • Changes in personal circumstances

  • Dissatisfaction with personal progress

  • Delayed HTMA collection

  • Incomplete participation

  • Failure to complete educational activities

  • Failure to implement educational suggestions

  • The Member’s choice to discontinue participation before completion of the 12-week program

I understand that enrollment reserves practitioner time and resources for my participation throughout the 12-week membership period.

12. SCHEDULING AND PARTICIPATION

The Rooted Life includes weekly private visits during the 12-week membership period unless otherwise agreed in writing.

I agree to provide reasonable advance notice when rescheduling is necessary.

Missed visits, missed calls, unused support time, late cancellations, or missed check-ins are not automatically:

  • Refunded

  • Credited

  • Rolled over

  • Added to the end of the program

The Association may offer rescheduling at its discretion and subject to availability, but rescheduling is not guaranteed.

The Association may communicate reasonable scheduling, cancellation, and rescheduling procedures separately.

When provided to the Member in writing, those procedures are incorporated into this Agreement.

I understand that delays caused by Member availability, delayed HTMA collection, shipping, laboratory processing, or other circumstances outside the Association’s reasonable control do not automatically extend the 12-week program.

13. COMMUNICATION BETWEEN SESSIONS

The Rooted Life includes a higher level of ongoing support than Rooted Foundations.

In addition to scheduled weekly sessions, the Member may receive up to one additional 15–20 minute support conversation each week, if needed and available.

This support is intended for:

  • Brief questions

  • Clarification

  • Discussion of information already addressed

  • Reasonable support between scheduled sessions

Additional support is subject to practitioner availability.

Unused support conversations do not accumulate, carry forward, create a credit, or extend the 12-week program.

Between-session communication does not provide unlimited texting, messaging, emailing, calls, or on-demand practitioner access.

Questions requiring extensive review, substantial new assessment, or significant additional practitioner time may be addressed during the next scheduled weekly session.

14. FOOD, HERBS, SUPPLEMENTS, AND WELLNESS PRACTICES

Educational information provided through The Rooted Life may include:

  • Food-based education

  • Meal ideas

  • Pantry education

  • Hydration education

  • Mineral education

  • Herbal education

  • Information regarding nonprescription supplements

  • Lifestyle practices

  • Movement

  • Sleep and stress-support practices

  • Other traditional or foundational wellness practices

Any suggestion is educational and is not medical nutrition therapy, a prescription, or a medical treatment plan.

I remain responsible for deciding whether to use any food, herb, supplement, or lifestyle practice discussed.

I understand that individuals may respond differently to foods, herbs, supplements, and wellness practices.

Allergies, sensitivities, medications, medical conditions, pregnancy, breastfeeding, and other individual circumstances may affect whether a particular product or practice is appropriate.

I am encouraged to consult an appropriately licensed healthcare professional or pharmacist regarding possible interactions, contraindications, medication decisions, or concerns requiring medical care.

15. CONFIDENTIALITY, PRIVACY, AND PRIVATE MATERIALS

I agree to maintain the privacy and confidentiality of the Association, its Members, private conversations, educational materials, documents, resources, recommendations, recordings, and communications.

Private Association materials are provided for my personal educational use.

I may not copy, publish, share, distribute, forward, sell, reproduce, teach from, or disclose private Association materials or Member information to a non-member without written permission from Major Roots Holistics.

This confidentiality responsibility continues after my Rooted Life membership ends.

Nothing in this provision prevents me from sharing this Agreement with legal, tax, medical, or other professional advisers for confidential review or from making disclosures required by applicable law.

The Association may collect and maintain information voluntarily provided by me for:

  • Membership administration

  • Scheduling

  • Educational preparation

  • Communication

  • Contribution or payment processing

  • HTMA coordination

  • Program support

  • Association recordkeeping

I understand that ordinary email and text messaging may carry inherent privacy risks and agree to communicate electronically when I choose those methods.

Consent to receive marketing messages is not required as a condition of membership.

Appointment reminders, scheduling communications, and program-related messages may be sent according to my stated communication preferences and applicable law.

16. VOLUNTARY PARTICIPATION AND ASSUMPTION OF RESPONSIBILITY

I acknowledge that I am voluntarily choosing to participate in The Rooted Life.

I accept responsibility for my personal choices, actions, food choices, lifestyle decisions, wellness practices, supplement choices, participation, outside care, and follow-through.

I understand that Major Roots Holistics provides private educational wellness information, guidance, accountability, and Member support.

I remain responsible for determining what choices are appropriate for me.

I understand that participation does not require perfection and that I may communicate when a suggestion or practice does not feel appropriate or realistic for my circumstances.

17. MEMBERSHIP CONDUCT, SUSPENSION, AND TERMINATION

Major Roots Holistics is committed to maintaining a respectful, supportive, and private Association environment.

Members agree to communicate respectfully and refrain from abusive, threatening, harassing, intentionally disruptive, or inappropriate conduct.

The Association may pause, suspend, or terminate membership for reasons including:

  • Nonpayment or failure to comply with an agreed contribution arrangement

  • Abusive or threatening conduct

  • Repeated violations of scheduling or participation expectations

  • Misuse or unauthorized sharing of private materials

  • Material dishonesty

  • Persistent requests for services outside the Association’s educational scope

  • Other substantial breach of this Agreement

When membership ends because of the Member’s breach, withdrawal, nonparticipation, or personal decision, the contribution remains non-refundable to the fullest extent permitted by applicable law.

If the Association becomes unable to provide the remaining portion of the program for reasons not caused by the Member, the Association will address unused services in a reasonable manner consistent with applicable law.

18. PRIVATE ASSOCIATION DISPUTE RESOLUTION

As a condition of private membership, the Member agrees that any concern, disagreement, or dispute arising from membership, participation, services, contributions, or this Agreement will first be addressed privately within Major Roots Holistics Private Membership Association.

The Member and the Association agree to make a good-faith effort to resolve the matter through direct communication and the Association’s internal dispute-resolution process.

If the matter cannot be resolved through direct communication, the parties agree to participate in private mediation or another mutually agreed-upon private resolution process.

The Member acknowledges that the purpose of this provision is to preserve the private nature of the Association and provide an orderly process for resolving disagreements between Members and the Association, to the fullest extent permitted by applicable law.

19. PRIVATE ASSOCIATION UNDERSTANDING AND GOVERNING TERMS

I understand that I am voluntarily entering into a private contractual relationship with Major Roots Holistics Private Membership Association.

I am requesting access to private educational wellness services as a Member of the Association.

I understand that my participation is based upon private membership and voluntary agreement between myself and the Association.

This membership relationship is entered into knowingly and voluntarily.

This Agreement is governed by the laws of the State of Wyoming, except to the extent that mandatory law in another jurisdiction applies.

The parties intend to resolve concerns according to the private Association dispute-resolution process described in this Agreement.

This Agreement, together with any incorporated written contribution, payment, or scheduling terms, represents the complete agreement concerning The Rooted Life and replaces prior oral or written statements regarding the same subject.

Any amendment to this Agreement must be in writing and accepted by both parties.

If any provision of this Agreement is found unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by applicable law.

Failure to enforce one provision does not waive the Association’s right to enforce that provision later.

20. ACKNOWLEDGMENT AND ACCEPTANCE

By signing or electronically accepting this Agreement, I acknowledge that I have read and understand its terms and have had the opportunity to ask questions.

I understand that The Rooted Life is a 12-week private educational wellness program offered through Major Roots Holistics Private Membership Association and includes one HTMA, weekly private sessions during the 12-week membership period, and up to one additional 15–20 minute support conversation per week when needed and available.

I understand that the membership contribution is $2,750.00.

I understand the educational nature of the services, contribution and no-refund policy, scheduling and participation expectations, confidentiality requirements, personal responsibilities, no-guarantee provisions, and private Association dispute-resolution process.

I understand that participation is voluntary, that I remain responsible for my own decisions, and that I may seek medical or other professional care whenever appropriate.

I voluntarily request membership participation in The Rooted Life and agree to the terms of this Agreement.

I agree that an electronic signature, checked acceptance box, or other electronic acceptance process associated with my identifying information may be used to show my acceptance of this Agreement.

Major Roots Holistics is a private wellness association offering educational wellness support. Information, guidance, and educational support are provided for lifestyle and wellness education purposes only and are not intended to diagnose, treat, or cure any condition or disease. Participation is voluntary and intended to support personal wellness choices and daily habits.
